THRIVE Outpatient Program
Transformative, Healing, Renewing, Inclusive, Voice, Empowerment
WellSpace Health - THRIVE Outpatient Program is a county funded program offering outpatient mental health care for adults with moderate to severe mental health challenges who are involved in the justice system. The program aims to break the cycle of repeated incarceration or hospitalization, helping individuals build healthier, stable lives in their communities.
Program Services
- Individual and/or group counseling
- Psychiatric services and medication management
- Coordination with court and justice partners
- Peer support services
- Strengths focused case management
- 24/7 Crisis intervention
- Support accessing physical health care and other wellness resources
- Skill building, social rehabilitation, and community connections
- Support with finding housing and rental subsidies
- Co-occurring support and linkage to substance use prevention and treatment services
- Reintegration supports following hospitalization and/or incarceration
Patients can access the program through referral from Sacramento County or through self referral.
Eligibility
- Sacramento County residents ages 18+
- Multi-system justice involvement
- Medi-Cal eligible
- Meet criteria for specialty mental health services
