WellSpace Health accepts a broad range of payment and insurance plans, including Medi-Cal and Medicare to cover the cost of health care services.
In addition, WellSpace Health operates on a sliding fee scale, which means the cost of services is based upon income and family size. No patient will be turned away based on their inability to pay. Our staff can also help patients enroll in free or low-cost insurance, such as Medi-Cal and Covered California, that can help cover the cost of treatment and services.
We make every possible effort to work with our patients regarding payments for their treatment and services.
Payment Policy Information
Payment is due at the time of service. All applicable co-insurance, co-payments, and deductibles are due at the time of service. An estimate will be provided; payment is required before services are rendered. This does not constitute final payment, and any additional balance due after the insurance claim is settled will be due upon receipt of a bill.
Physicians’ Open Payments Notice
The Open Payments database is a federal tool used to search for payments by drug and device companies to physicians and teaching hospitals. This database can be found at https://openpaymentsdata.cms.gov.
A link to the federal Centers for Medicare and Medicaid Services (CMS) Open Payments web page is provided for informational purposes only. The federal Physician Payments Sunshine Act requires that detailed information about payment and other payments of value worth over ten dollars ($10) from manufacturers of drugs, medical devices, and biologics to physicians and teaching hospitals be made available to the public.
Good Faith Estimate Notice
Disclaimer: This Good Faith Estimate shows the costs of items and services that are reasonably expected for your health care needs for an item or service. The estimate is based on information known when the estimate was created. The Good Faith Estimate does not include any unknown or unexpected costs that may arise during treatment. You could be charged more if complications or special circumstances occur. Federal law allows you to dispute (appeal) the bill if this happens. We will expect payment at the time of service.
Sliding Scale Program
WellSpace Health is committed to serving all patients/clients, regardless of their ability to pay. Although we accept many healthcare coverage programs, patients/clients not covered by insurance will not be turned away.
If you are uninsured and it is determined that you are not eligible for public insurance programs like Medi-Cal, you may be eligible for a Sliding Fee Scale program discount. To apply for the Sliding Fee Scale program, you must provide proof of income and sign the affidavit below. All information for this voluntary program is confidential. If you do not complete the Program Screening Application (including Proof of Income), you are welcome to be a private pay patient at 100% of the cost.

You must provide Proof of Income within 30 days from the date of application to be eligible for future sliding fee scale discounts. If you do not provide Proof of Income within 30 days, you are welcome to be a private pay patient at 100% of the cost. Payment is due at the time of service. Your eligibility for the Sliding Fee Scale program is good for 12 months and must be renewed annually.
One month’s documentation of any and all of the following sources of income:
- Paycheck stubs/statements less than 60 days old
- Alimony checks/statements
- Signed Statement from employer (if paid in cash)
- Interest and/or dividend income statements
- Workers Compensation
- SDI, Social Security, Unemployment
- Pension check stubs/statements
- Current federal income tax return
List of Accepted Insurance
WellSpace Health accepts various coverage options, including but not limited to Medi-Cal, Medi-Cal Managed Care, Medicare, Child Health and Disability Prevention Program (CHDP), Family PACT, Every Woman Counts, some commercial employer plans, and self-pay payments on a sliding fee scale. You are advised to always check with your insurance plan to confirm if they contract with WellSpace Health directly.
