Care Transitions are programs supporting individuals transitioning from one model of care to another. Care Transition programs are characterized by shorter term interventions seeking to connect a person to consistent ongoing healthcare services and a health home.

Street Medicine

Street Medicine deploys nurses to local neighborhoods to provide care to highly vulnerable people who are experiencing homelessness and are not accessing necessary primary care services.

Recuperative Care (Medical Respite)

Recuperative Care (medical respite) provides care for people who no longer require hospitalization but still need to heal from an injury or illness exacerbated by an experience of homelessness.

Short Term Post Hospitalization

Short Term Post Hospitalization provides people who do not have a residence and who have high medical and/or behavioral health needs with the opportunity to continue their recovery after existing an immediately after exiting an inpatient hospital, treatment center, correctional facility, or recuperative care.

Transitions Clinic Network

Transitions Clinic Network improves the health and reentry outcomes for people returning from incarceration caring for a patient’s physical and behavioral health needs, and their social determinants of health.
