Flexible Case Management promotes client success through one-on-one planning and coordination to implement an individual care plan and connection to supportive benefits and services.

Flexible Case management includes Enhanced Care Management, Emergency Department Navigators, T3 (Triage, Transport, Treat), and Housing and Retention programs.

Services

Enhanced Care Management

Enhanced Care Management addresses clinical and non-clinical needs of the highest-need people through intensive coordination of health and health-related services. It meets a persons needs wherever they are – on the street, in a shelter, in their doctor's office, or at home. A person will have a single Lead Care Manager who will coordinate care and services among the physical, behavioral, dental, developmental, and social services delivery systems, making it easier for them to get the right care at the right time.

Emergency Department Navigators

Emergency Department Navigators are community health workers who help connect people in emergency departments to care and services to address their health and social determinants of health.

T3 (Triage, Transport, Treat)
T3 – Triage, Transport, Treat is a hospital-based program identifying high utilizers of emergency departments and facilitates care in a primary care setting when appropriate.

Housing and Retention

Housing and Retention provides supportive services within Permanent Supportive Housing to help highly vulnerable clients access and maintain safe and stable housing. To access housing, clients are referred to community partners who specialize in housing access.
