How to Prepare for an Appointment
In order to provide you the best possible care, your WellSpace Health provider will likely ask to review your previous medical records, if available. You may have your records faxed by your previous primary care provider to (916) 313-8452 or bring them with you. Please arrive at least 30 minutes prior to your first appointment in order to complete your patient registration.
What to Bring
Always bring your Medi-Cal or Medicare card, or other proof of insurance when you visit one of our health centers. If it is your first visit, please bring identification (photo identification, such as a valid driver’s license, birth certificate or other form of official identification), proof of residency, proof of income, and current medications. If you are uninsured, you are not required to provide proof of insurance.
