Preparing for your first appointment

In order to provide you the best possible care, your WellSpace Health provider will likely ask to review your previous medical records, if available. You may have your records faxed by your previous primary care provider to (916) 313-8452 or bring them with you. Please arrive at least 30 minutes prior to your first appointment in order to complete your patient registration.

What to bring to your appointment

Always bring your Medi-Cal or Medicare card when you visit one of our health centers. If it is your first visit, please bring proper identification, such as a valid driver’s license, birth certificate or other form of official identification.

If you have insurance

WellSpace Health accepts Medi-Cal and Medicare insurance. If you do not currently have either of these and do not know if you qualify, our friendly health center staff will put you in touch with outreach workers who can assist you.

If you do not have insurance, you may qualify for a sliding fee scale. We request that you provide a pay stub, an unemployment check stub, a recent tax return or a letter from your employer in order to prove total family income and family size. Phone or utility bills are required for proof of California residency.
